Paperwork and Birth Packets

For each certifying birth, you will remit a paperwork packet including:
Demographic Sheet (with client release signed)
Intake Form and Birth Preferences
Notes for first and second prenatal meeting
Notes from birth support
Postpartum visit note
Post Birth Evaluation
 
You will connect to a local mentor doula for a minimum of 12 months. Once you third certifying birth has been completed, you will remit a doula recommendation for certification from your doula mentor before achieving certification status. 
 
Certification can be received once all reqiurements are fulfilled and a $50 certification fee is paid.
 
Certification is good for two years. Certification can be renewed after completing one additional birth in the previous twelve months and remitting a birth packet. 20 CEUs in OB, Pregnancy, Birth, Postpartum, or Newborn are required. Recertification fee is $50.
